Health Partners Plans ofrece recursos comunitarios gratuitos y de costo reducido con la tía Bertha

Philadelphia, PA — Health Partners Plans (HPP) has partnered with Aunt Bertha, a network that connects people seeking help with verified social care providers to serve them. The partnership allows HPP members to search online for free or reduced-cost help around food, housing, transit, job training and more.

As part of this partnership, HPP recently launched Your Partner in Finding Resources, which is available to HPP staff, members, providers and community-based organizations (CBOs), in partnership with Aunt Bertha. This resource can be used to address upstream issues and Social Determinants of Health (SDoH) including food insecurity, workforce support, housing navigation and more. For many people, navigating the system to receive help can be difficult, but Aunt Bertha makes it easy for people to find the resources they need.

“HPP sees managed care organizations as having a responsibility to look at all aspects of members’ lives that impact their health goals and their overall health outcomes,” said Kristin Beck, director of community health programs at HPP. “These organizations are trusted resources for our members and these services allow us to address our members' health needs and to help improve their quality of life. HPP’s partnership with Aunt Bertha demonstrates our commitment to quickly linking members with the resources they need and facilitating solutions to the challenges they face on a daily basis.”

Starting in July, HPP will host training events for CBOs to share how to implement the Aunt Bertha platform and best utilize it to help more people. These monthly community-based trainings are free and open to community stakeholders. Through these trainings, HPP hopes to get more programs added to the Aunt Bertha platform and ultimately direct more people to programs that can help, while empowering them to take charge of their health.

Providing essential services through coordination with local organizations leads to improved support and services pre crisis. Aunt Bertha provides real-time data to ensure that needs are being met long before a member interacts with the health care system, as well as provides analysis to support planning and policy development. The Aunt Bertha platform enables HPP to track ongoing interactions between SDoH partners and members.
