Our Enhanced Member Supports Unit (EMSU) is here to help you get the care and services you need to stay healthy. Our supportive Care Coordinators work with your doctors to connect you to the right resources. Whether you need help managing complex care, caring for a sick child, preparing for a new baby, or connecting with resources for health-related social needs, we’re here for you.
Baby Partners: Helps expecting and new mothers get the care they need.
Pediatric Care Coordination: Assists parents and guardians connect to resources and coordinate health care services to support their child’s overall well-being.
Adult Care Coordination: Supports members 21 and older with their health needs, including complex care.
A Care Coordinator is a social worker or nurse who can help you:
We offer these programs to members who may need some extra support. You aren’t required to participate, and you can stop at any time. Opting out will not affect your Health Partners Plans Medicaid benefits.
