HPP would like to inform you about a recent update to the blood pressure cuff order form. The goal of updating the form is to reduce the administrative work for you and your team.

As a reminder, both Medicaid and Medicare members have benefits for a blood pressure cuff. The blood pressure cuff order form can be accessed at www.HPPlans.com/cuff.

Updates and helpful tips:

• Please start to use the new form immediately.
• The updated form can be used for an individual patient or as a bulk order form. Please fill out the form completely, sign it and attach a list of ALL members that need a BP cuff.
• Email OR fax the completed form to the DME company.

• HPP recently distributed aCBP Education and Resource Guide. The guide can be found here: www.HPPlans.com/BloodPressure.
• Patient-reported blood pressure readings during telehealth visits or telephone assessments are permissible.
