ALTERNATE OPTION
While participation in this webinar will satisfy your annual HPP training requirement, there is an alternative method to complete the required training.
Providers can download an electronic copy of the training at www.healthpartnersplans.com/providers/training-and-education/required-training (under the “Annual Training Requirement” header) to review with your staff. Simply complete and submit the electronic attestation found at the end of the training.
D-SNP MODEL OF CARE TRAINING
Successful completion of the D-SNP Model of Care training module is mandatory for providers serving HPP Medicare-Dual Eligible Special Needs Plan (D-SNP) members. At least one member of a care team location is required to take the annual online training course and distribute the training material to all D-SNP care team members.
To take the training, go to www.healthpartnersplans.com/providers/training-and-education/required-training and complete the attestation upon completing the training.
ACCESS AND APPOINTMENT & AVAILABILITY STANDARDS
Timely access to quality health care is extremely important for our members. As you should be aware, it is a regulatory requirement that all HPP-participating providers must meet clearly defined access, appointment and availability standards. These standards can be found in Chapter 10, Provider Practice Standards and Guidelines, of the HPP Provider Manual, available at hpplans.com/manual.
IDENTIFYING DEFICIENCIES: HPP SURVEYS
Each year, HPP surveys our providers to determine if they are meeting these standards. A new survey is planned for 2022. If the survey reveals that your office is found to be deficient in meeting the required access, appointment and/or availability standards, your office will be sent a letter identifying the deficiency. A follow-up survey will then be conducted within a few months; if the deficient office has not shown adequate improvement, HPP will initiate a process improvement plan to correct the deficiency.
