Medicaid Diabetes Prevention Program (DPP) Reminder and New Incentive
Starting in 2022, Health Partners Medicaid members who attend at least four Diabetic Prevention Program (DPP) core sessions are eligible to receive a free one-month membership to Weight Watchers of Philadelphia. This is an added incentive to encourage our eligible members to participate in a DPP.
Diabetes Prevention Program (DPP) Reminder
Medicaid patients with prediabetes or risk factors for diabetes may benefit from the Centers for Disease Control & Prevention (CDC) diabetes prevention lifestyle change program. Patients may be eligible to participate in this program based on these eligibility requirements:
- Be 18 years or older, and
- Have a body mass index (BMI) of 25 or higher (23 or higher if you are Asian American), and
- Not be previously diagnosed with type 1 or type 2 diabetes, and
- Not be pregnant.
Patients will also need to meet 1 of these requirements:
- Had a blood test result in the prediabetes range within the past year (includes any of these tests and results):
- Hemoglobin A1C: 5.7–6.4%, or
- Fasting plasma glucose: 110–125 mg/dL, or
- 2-hour plasma glucose (after a 75 g glucose load): 140–199 mg/dL, or
- Be previously diagnosed with gestational diabetes (diabetes during pregnancy), or
- Received a high-risk result (score of 5 or higher) on the Prediabetes Risk Test.
DPPs help at-risk individuals make healthier lifestyle choices and may prevent diabetes from developing through weight loss, a healthier diet, stress reduction and increased physical activity. There are 25 one-hour sessions delivered over the course of a year. Some sessions are being offered virtually. For more information or to see a list of DPP suppliers, visit hpplans.com/dpp.
