Access and Availability Survey Reminder
Thank you for your participation in the Jefferson Health Plans network and helping to meet the medical needs of our members.
Each year, Jefferson Health Plans surveys our network to determine if primary care and specialist providers are meeting the access, appointment, and telephone availability standards, as set by the Department of Human Services (DHS) for Health Partners (Medicaid) and KidzPartners (CHIP) members, and by the Centers for Medicare & Medicaid Services (CMS) for Health Partners Medicare members.
Our provider access, appointment and telephone availability standards survey is now available online. We ask that you complete the survey using your unique survey ID number (shown below) by October 5, 2023. Completion of the survey is mandatory for all participating provider groups. We require one survey to be completed for each survey ID number, which equates to each provider’s office location.

Please answer the survey with a current representation of your ability to get members in for care. It is our expectation that all participating provider offices comply with the latest access and availability standards. However, if the survey results reveal that your office is not meeting the required access, appointment, and telephone availability standards, we will follow up with your office and identify the area(s) of concern and collaborate with you to ensure your office can meet these important standards.
