MQCP is designed to recognize and reward your practice’s performance in delivering high-quality services throughout the year. Every year, we assess the program to ensure a positive impact on both our members and our provider network.

The 2024 MQCP Manual is now available on the provider portal and at HPPlans.com/MQCP. Please note that the manual will not be mailed to your office.

 In October 2023, we shared updates to the 2024 MQCP program with our provider network. Details of those updates can be found in this manual.

As a reminder, ONAF has been removed as an MQCP program measure and is now a separate reimbursement program. Additional details about this program can be found at www.healthpartnersplans.com/providers/quality-and-population-health/pay-for-performance-qcp-and-mqcp.
